同樣是尼古丁載體，加熱菸與紙菸在「燃燒」與「加熱」兩字背後，決定了化學反應與健康風險的巨大落差。香菸透過600°C以上高溫燃燒，產生焦油及數千種燃燒副產物，是慢性阻塞性肺病與肺癌的主要推手。

加熱煙則以電子裝置將特製菸草加熱至350–500°C，利用揮發與熱裂解釋放尼古丁，理論上減少焦油總量，卻仍產生甲醛、乙醛、亞硝胺等有毒物質。

使用方式上，前者點火即抽，後者得充電、插入專用加熱菸彈，步驟繁瑣；煙霧氣味方面，IQOS因溫度較低，殘留氣味較淡，但依舊有二手菸與三手菸問題。值得注意的是，兩者皆含高度成癮的尼古丁，皆非戒菸工具。
